Job Description
As a Street Outreach Advisor, you will work directly in the community to connect individuals with care and support services that improve health and stability. You will identify and prioritize community-based organizations, build and maintain referral-generating partnerships, and spend most of your workweek in the field meeting with potential partners and engaging the community through in-person outreach.
Requirements
- Experience in community outreach, public health, social services, healthcare navigation, or similar field-based work
- Comfort working independently in a fast-paced, community-facing environment
- Ability to manage outreach activity, relationships, and follow-through across multiple partners
- Strong organizational skills and comfort using mobile tools and CRM systems
- Valid driver’s license and ability to travel locally
