Job Description
Partners Community Health is seeking a Nurse Practitioner to provide direct, comprehensive care to long-term care facility residents with specialized and complex needs.
Requirements
- Bachelor's Degree in Nursing; Master's in Nursing preferred
- Completion of the Ontario Primary Health Care Nurse Practitioner Certificate Program
- Current registration with the College of Nurses of Ontario as a Registered Nurse in the Extended Class
- Membership with the Nurse Practitioners' Association of Ontario (NPAO) and Registered Nurses' Association of Ontario (RNAO)
- Canadian Gerontological Nursing certification or plans to obtain certification
- Minimum 5 years' experience in gerontological nursing or primary health care as a Nurse Practitioner (preferred)
- Experience in geriatrics and/or geriatric psychiatry, particularly in managing Behavioural and Psychological Symptoms of Dementia (BPSD) and responsive behaviours (preferred)
- Experience working in long-term care settings
