Job Description
This Nurse Case Manager I position involves initiating and coordinating multidisciplinary team approaches to case management, focusing on patient health and wellness goals. The role entails individual care plan development, communication with providers and member representatives, case management systems management, and documentation. The position requires a commitment to evidence-based interventions and a collaborative team environment.
Requirements
- Associate’s degree in Nursing required; Bachelor’s degree in Nursing(or higher) preferred.
- 2 years of experience in case management (1+ year acute care preferred)
- Current and unrestricted California Registered Nurse License.
- ValidCalifornia driver’s license and proof of current automobile insurance
- Bilingual skills in Spanish, Russian, or Tagalog preferred.
