Job Description
PATH is seeking a Clinical Case Manager to join their Site Based Housing team. The Case Manager will provide field-based, direct client outreach, linkage and retention services to veterans experiencing homelessness.
Requirements
- Master’s Degree in Social Work or equivalent behavioral health degree
- One year of experience in healthcare, case management, homeless services or working with vulnerable populations
- Current and active licensure or registration with the California Board of Behavioral Sciences (e.g., ACSW, LCSW)
- Employment eligibility verification
- CPR/First Aid training
- Background Screening
- Drug Test
- Reliable transportation
- A valid driver’s license
- Proof of insurance and ownership for personal vehicles used during work duties
- The ability to qualify for PATH's insurance coverage
