Complex Care Coordinator (LSW)

PHMC is seeking a Licensed Social Worker (LSW) to work as a Complex Care Coordinator. The LSW will provide community-based clinical social work, case management and outreach services to diverse and underserved patient populations.

Requirements

• Provide multidisciplinary assessments to determine needs and barriers in healthcare, social supports, and access to behavioral health services
• Actively participate in the interdisciplinary healthcare team and provide input and strategies to address health, behavioral health, and psychosocial issues
• Collaborate with medical and auxiliary providers to develop healthcare plans and goals
• Provide outreach and engage with clients to promote and encourage self-care
• Ensure timely and coordinated access to health and social services by promoting and modeling advocacy skills
• Document all services in a timely manner in the electronic health record system
