Public Health Management Corporation (PHMC) is a nonprofit public health institute serving the Greater Philadelphia region since 1972. The Social Worker will provide services to clients at the PHMC Health Network, assessing their social determinants of health and connecting them to social service supports and resources. They will also provide information about community resources and work closely with the health care team to coordinate client care and improve health outcomes.
