Job Description
Phoenix Home Care and Hospice is seeking a compassionate and skilled Home Health LPN to join our team, delivering high-quality nursing care in the comfort of our clients' homes, working closely with a team of healthcare professionals, and embodying our core values of compassion and integrity.
Requirements
- Current LPN license in the state of practice
- Minimum of one year of nursing experience, preferably in home health or community health
- Compassionate and patient-centered approach to care
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
- Basic computer skills and familiarity with electronic health records
