Home Health LPN

Phoenix Home Care and Hospice is seeking a compassionate and skilled Home Health LPN to join their team, providing high-quality nursing care to clients in the comfort of their homes.

Requirements

• Current LPN license in the state of practice
• Minimum of one year of nursing experience, preferably in home health or community health
• Compassionate and patient-centered approach to care
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Basic computer skills and familiarity with electronic health records
• Willingness to undergo background checks and health screenings
