Job Description
Porter Cares is expanding into Michigan and seeking Nurse Practitioners to provide in-home health risk assessments. The role involves evaluating patient history, conducting physical exams, and identifying diagnoses to support care management and treatment, contributing to a transparent and personalized healthcare approach.
Requirements
- Valid unencumbered Michigan NP license
- 3 years of patient care experience required
- Experience in primary care/adult/geriatric preferred
- EMR skills preferred
- Strong sense of empathy, compassion, and friendliness
- Exceptional communication skills
