Job Description
Porter is seeking nurse practitioners to serve areas in Winnebago and Rockford, Illinois. The role involves conducting in-home health risk assessments, evaluating medical history, performing physical exams, and identifying diagnoses to support care management and treatment. Requirements
- Valid unencumbered Illinois NP license
- 3 years of patient care experience required
- Experience in primary care/adult/geriatric preferred
- EMR skills preferred
- A strong sense of empathy, compassion, and friendliness
- Exceptional communication skills, both written and oral
