Providence Health and Rehabilitation is seeking a dynamic Director of Care Management to lead and optimize care management processes for St. Peter Hospital and Centralia Hospital. This role is pivotal in ensuring a seamless continuum of care for patients, advocating for patient needs, and managing resources efficiently. The Director will oversee a dedicated team and work closely with the North Division Executive Director to maintain financial viability and ensure a patient-centric environment.
