Job Description
This Mental Health Targeted Case Manager position involves managing up to 25 clients, creating individualized care plans, and ensuring the safety and well-being of the clients. The role requires collaboration with therapists, families, and community partners, and adheres to agency protocols and regulations. TCMs will ensure the delivery of services appropriate to individual client needs.
Requirements
- Bachelor’s Degree in a human services or related field
- One year of experience in community-based services ( postgraduate or Master’s Degree)
- Strong communication and conflict resolution skills
- Valid Driver’s License and insurability
- Proficiency with Microsoft Office Suite
- Understanding and relating to diverse client needs
- Ability to understand English
