Job Description
This pharmacist position at Westfields Hospital & Clinic requires a collaborative member of the team to enhance medication outcomes. Requirements
- Current Wisconsin pharmacist license (or eligibility)
- Admitted to accredited college of pharmacy program (or completed program)
- Two years of MTM service experience in a multidisciplinary team
- Successful completion of a one-year post-doctoral residency or a structured education program (or post-doctoral residency approved by NCQA)
- Credentialing required through NCQA guidelines.
