Job Description
Westfields Hospital & Clinic is seeking a Pharmacist to deliver clinical pharmacy services from Westfields Hospital & Clinic. The role focuses on enhancing medication outcomes by collaborating with a multidisciplinary medical team in an ambulatory setting. This position ensures safe and effective drug therapy and prevents or mitigates disease, while also improving patient quality of life.
Requirements
- Current Wisconsin pharmacist license or eligibility
- Graduation from an accredited college of pharmacy (after May 1996 or structured education program)
- Two years of MTM service experience in a multidisciplinary team
- Successful completion of a one-year post-doctoral residency in ambulatory care pharmacy services (optional).
