Job Description
Samaritan Daytop Village is seeking a Case Manager to work directly with individuals experiencing homelessness in Queens, NY. The role involves conducting client interviews, developing Independent Living Plans, coordinating referrals to support services, and advocating for client stability. Case Management focuses on identifying resources and helping clients overcome barriers to housing and self-sufficiency.
Requirements
- Build professional relationships with clients and maintain confidentiality
- Review client eligibility documents and explore alternatives
- Create and manage client files and records
- Conduct intake interviews and psycho-social assessments
- Develop and update Independent Living Plans
- Refer clients to permanent housing and community-based services
- Support clients in overcoming barriers and advocating for themselves
- Track client progress and document updates
- Follow up with clients and referral agencies
- Provide data for required reports
