PRN Case Manager

The Case Manager is responsible for managing an assigned caseload of clients with mental illness or substance abuse and assessing client needs, developing service plans, and working with community resources. The Case Manager will also provide supportive case management functions, document service contacts, and access daily admission reports to effectively meet new patients.

Requirements

• Bachelor's degree in Social Work or Social Services
• Two years' experience in healthcare or healthcare-related case management or coordination duties
• Valid CPR certification and SAMA training
• Bi-lingual English/Spanish preferred but not required
