About the unit: Our sub-acute floor is a 59 bed Medicare unit that has residents with multiple clinical issues. Our Chronic Care floor is a 60 bed Medicare/Rehabilitation and long-term care unit. Our Dementia Care floor is a 60 bed Medicare/Medicaid/Dementia unit. About the Facility: Our facility has 3 floors and 9 units, providing care for long term care residents and post-acute rehab patients.

Requirements

• Acts as a team member in the delivery of quality, direct patient care.
• Performs basic skills per department guidelines and communicates observations to members of the health care team.
• Provides direct patient care under the direction of the Unit Manager or Licensed Nurse, consistent with policies and procedures of the unit.
