Job Description
Strive Health is seeking a Medical Director, Care Management to lead the design, implementation, and effectiveness of transitional care management, intensive care management, and utilization management clinical programs. The ideal candidate will have 6+ years of experience in cardiology and kidney care management, including experience with CHF, CKD, and ESKD populations.
Requirements
- 6+ years experience working in cardiology and kidney care management
- Current state-specific licensure
- Current Drug Enforcement Administration (DEA) license
- Current BLS or CPR Certification
- Board-certified in Nephrology, Internal Medicine, Cardiology, Family Medicine, Emergency Medicine, Geriatric Medicine, Hospice and Palliative Medicine, or another relevant specialty
