Patient Coordinator

Requirements

• Answer and respond to patient phone calls
• Manage team voicemail and email
• Triage patient phone calls
• Schedule and logistics for urgent need patients
• Coordinate patient care between team members and other service line teams
• Communicate information to patients and staff
• Maintain worklist and enter physician orders in electronic medical record
• Complete patient forms and provide information packets as directed
