MDS Coordinator, RN

Sunnyside Nursing and Post-Acute Care is seeking a Registered Nurse to conduct and coordinate resident MD/RAI/CAAS documentation and monitoring, verify assessments, and assist in the planning of training. The role involves assisting with resident care plans, participating in committees, and ensuring quality in all aspects of patient care and documentation. This position focuses on providing compassionate and individualized care in a supportive, home-like environment.

Requirements

• Must possess a Nursing Degree or be a graduate of an approved Registered Nursing Program
• Must have an active, unencumbered CA license
• Must be RAC-CT or eligible within 30 days of hire
• Must be knowledgeable of CASPER-related information
• Must be knowledgeable of WQIP processes and scores
• PCC and Simple LTC experience
• Must be authorized to work in the United States
