Care Manager

Sunrise Senior Living is seeking a Care Manager/Designated Care Manager to provide high-quality care and services to a group of residents in their assisted living and reminiscence neighborhoods. The role involves resident care, activities of daily living assistance, ensuring safety and promoting dignity, and communication with families. Requirements

• Review and note resident information.
• Participate in the development of individual service plans (ISPs).
• Respond to resident needs and ensure safety.
• Maintain a safe and comfortable home environment.
• Engage residents in reminiscence and activities.
