Sunrise Senior Living is seeking a Care Manager/Designated Care Manager to provide high-quality care and services to residents in assisted living and reminiscence neighborhoods. This role involves resident care, maintaining a safe environment, and promoting independence and dignity. The Care Manager will implement and adhere to residents' Individualized Service Plans (ISPs) and participate in team meetings.

Requirements

• Participate in the development and update of Resident Improvement Plans.
