Case Manager

Requirements

• Provide case management to assigned caseload
• Develop and monitor individual service plans in conjunction with agency consumers
• Assist agency consumers with accessing needed community services
• Act as an advocate for consumer
• Maintain a strong working relationship with therapeutic support services
• Document all services in compliance with agency standards
• Participate in ongoing supervision and team meetings
