Job Description
The Role: Discharge Planner-RN - Main Case Management - Part Time - Days. The RN Case Manager will ensure patients receive medically necessary, evidence-based care, following the Integrated Case Management Model. This involves clinical review of 100% acute bedded patients, discharge planning, and proactive identification of patient needs with social services. The role involves collaboration with external agencies and healthcare facilities.
