Job Description
The RN/Case Manager role at Mount Sinai Health System involves managing assigned patients to determine care needs, implement care plans, and monitor patient length of stay. Requirements
- 1. Reviews new admissions to identify patients requiring level of care and discharge planning.
- 2. Collaborates with physician and other clinicians to expedite diagnostic testing and consultations.
- 3. Documents all clinical reviews in Case Management Documentation system.
- 4. Maintains a working knowledge of Utilization Review requirements.
- 5. Assists with discharge planning, including assessments, authorizations, and liaison with payers.
