Job Description
As an Outpatient Diabetes Educator, you will be part of a multi-disciplinary team responsible for designing, coordinating, and implementing optimal diabetes care and education. This role involves providing comprehensive training, monitoring patient progress, collaborating with other healthcare providers, and contributing to the diabetes education program within The Christ Hospital. This position requires strong communication and teamwork skills.
Requirements
- Comprehensive diabetes self-management training.
- Assessment of patient needs for diabetes and families.
- Guidance in goal determination and program implementation.
- Patient monitoring and progress tracking.
- Diabetes self-management training in accordance with ADA standards.
