The Inpatient Cert Diabetes Educator provides timely and targeted education and support services to patients and support system, bridging the gap between clinical care and self-management.
Requirements
- Bridges the gap between clinical care and self-mgmt.
- Delivering survival skills education, ensures a smooth transition from inpatient to outpatient.
- Offers guidance to medical staff and participates in regular and ad-hoc education initiatives.
- Works collaboratively with interprofessional teams.
- Facilitates monthly diabetes support group.
