Job Description
Colorado PACE has an immediate opening for a LPN, full-time hybrid, Home Care and Day Center. The position involves providing patient home health care in accordance with applicable scope and standards of practice, and collaborating with RN Case Manager, Charge RN or assigned RN in the coordination of care.
Requirements
- Graduate of an accredited school of practical nursing.
- One year of experience in hospice, palliative care and/or home health, medical/surgical, public health, oncology, acute care or home health.
- Current Colorado LPN license.
- Current CPR certification.
- Valid Colorado driver’s license and proof of automobile insurance in accordance with state statute.
