Job Description
The Excalibur Group is seeking a Support Broker/Case Manager in Washington, DC to support a local contract. The position involves coordinating Medicaid Waiver services, promoting personal growth and independence, and ensuring quality and timeliness of documentation.
Requirements
- Be 18 years of age or older
- Be a DC Resident
- Have current, appropriate licensure
- Have a Master degree in Social Work, Psychology, Counseling, Rehabilitation, Nursing Gerontology or Sociology
- Have one (1) year experience with working with elderly population and people with disabilities
- Successfully complete all required criminal background check(s) pursuant to the State of Maine and federal requirements
- Excellent knowledge of habilitation and personal assistance scope of services, state specific regulations and applicable regulation standards, and applicable accreditation standards
- Excellent interpersonal skills
- Ability to manage a schedule and frequent travel
- Strong desire to help individuals live a life of their choosing
- Strong networking skills; knowledge of the local community; ability to connect people to community resources
Benefits
- $55,000 annual salary
- Flexible work schedule (Mon. – Fri. 8:30am – 5pm, except Federal Holidays)
