Care Manager RN - University Hospital/Ross Heart Hospital

The Clinical Case Manager functions within the multidisciplinary team to facilitate an assigned group of patients through the health care system with the primary focus of efficient, effective, compassionate inpatient care and a safe transition to the next level of care.

Requirements

• RN license required, BSN preferred or Licensed Masters SW with previous CM and/or discharge planning experience
• 2 years clinical experience applicable to the patient population, home care, hospice, community agency, and/or payer relations required
• Prior case management experience strongly preferred
