Job Description
Theoria Medical is seeking a Part-time Nurse Practitioner to provide primary care to residents in Skilled Nursing Facilities (SNFs). This position offers flexible schedules, no on-call requirements, and a supportive community. The role involves rounding, prescribing medications, and providing patient-centered care through the EHR system, ChartEasyTM.
Requirements
- Rounding and managing clinical care of the patient population at assigned SNF
- Prescribing medications and treatments for SNF residents
- Providing high-quality, patient-centered care in SNFs
- Timely documentation of all patient care
- Reliable transportation
- Availability for phone/secure messaging consultations with facility nursing staff
