Job Description
Theoria Medical is seeking a Part-time Nurse Practitioner to provide primary care services to residents in Skilled Nursing Facilities (SNFs). The role offers flexible schedules, no on-call or overnight requirements, and a supportive, flexible, and appreciative work environment. The EHR system is Keypoint EMR, PointClickCare, MatrixCare, and American HealthTech.
Requirements
- Provider is responsible for rounding and managing clinical care of the population at the assigned SNF.
- Provider is responsible for rounding daily and prescribing medications and treatments.
- Provider will provide high quality, patient-centered, comprehensive primary care services to patients residing in SNFs.
- Provider will be available for phone/secure messaging consultations with facility nursing staff.
- Provider documentation is completed in ChartEasyTM, PointClickCare, MatrixCare, and American HealthTech.
