The Permanent Supportive Housing Case Manager supports HTH residents in accessing support services, community integration, life skill training, housing maintenance, transportation, and other needed services. This role provides day-to-day case management for residents who are transitioning from homelessness to safe, secure, long-term housing.
Requirements
- 2+ years of experience in social services case management
- High School Diploma or GED; Bachelor’s degree in Social Work, Psychology, or related human service field preferred
- Knowledge of HIPAA compliance when collecting and storing participant's information
- Knowledge and ability to access entitlement programs, community, social, medical, mental, and chemical dependency services and/or education, job training, and employment services within ULMS and outside the organization as appropriate based on client(s) need
- Knowledge and ability to successfully implement interviewing, counseling methods, principles, and techniques that result in measurable positive outcomes
- Ability to analyze interventions for effectiveness, research and develop alternative interventions, and openly solicit supervisory guidance and input
- Proven ability to maintain accurate, thorough, timely, and complete case files and ability to communicate professionally in writing and verbally; Knowledge of electronic recordkeeping for client information
- Ability to work with diverse populations (language, culture, race, physical ability, sexual orientation, etc.)
- Excellent documentation and time management skills
- Proven ability to build relationships and trust quickly
