Intake Therapist - Adult Outpatient Services | Valley Access

We are seeking an Intake Therapist for our Adult Outpatient Services program in Salt Lake City, UT. As an Intake Therapist, you will conduct intake assessments and ASAMs for incoming clients, develop and implement personalized care plans, and collaborate with multidisciplinary teams to prioritize client care.

Requirements

• Master’s degree in Social Work, Clinical Mental Health Counseling, Marriage and Family Therapy, or related field
• ACMHC, AMFT, CSW, licensure in the State of Utah
• CMHC, LMFT, LCSW licensure in the State of Utah
• Valley de-escalation certification (provided)
• Prior experience treating adults with SUD and/or mental health disorders (preferred)
