Job Description

Requirements
- Ability to travel.
- Thorough knowledge of community resources.
- Knowledge of psychosocial issues in terminally ill patients.
- Excellent communication and interpersonal skills.
- Master's of Social Work and a valid license (LMSW, LGSW, LSW) or equivalent
- At least one year of social work experience in a health care setting.
