Job Description
WelbeHealth is seeking a Primary Care Physician with a Pod Leader role to provide comprehensive, community-based care to seniors with a focus on building long-term relationships. As a PACE program, the role focuses on holistic care, offering significant potential for growth and work-life balance. They are looking for a physician to perform in-person assessments, coordinate discharge planning, and provide care in a fast-paced environment.
Requirements
- Perform comprehensive in-person assessments
- Coordinate discharge for acute, short-term, and long-term placement
- Be eligible for enrollment to provide services to Medicare and Medi-Cal participants
- Board certification preferred
