Job Description
WellBe Senior Medical is seeking a highly skilled Nurse Practitioner to provide home-based medical visits, deliver primary care, and manage the health and wellbeing of patients in their homes. Requirements
- Master’s Degree in Nursing with focus on Family Nurse Practitioner, Adult Nurse Practitioner, or Geriatric Nurse Practitioner
- Active, unencumbered Nurse Practitioner license in the state of job posting
- Active, unencumbered Nurse Practitioner license in the state of hire
- Current board certification in either ANCC or AANP
- BLS certified and Active NPI# as an NP
- CPR certification
- Current DEA licensure or eligibility
- Valid driver’s license, car insurance, and willingness to drive to patient homes
- Proof of an active auto insurance policy
- Experience in value-based care, home care, primary care, hospice, palliative care, or geriatrics (preferred)
- Telehealth experience (preferred)
- Home visits required
- Demonstrated proficiency with Microsoft Office applications.
