Clinical Case Manager I/II - High Intensity Treatment Team

Clinical Case Manager provides comprehensive psychological and behavioral case management services within a continuum of high to low intensity services. They assess, monitor, and follow up with consumers' ability to meet basic needs and provide transportation to needed services and community resources.

Requirements

• Bachelor (CM I) or Master (CM II) Degree in psychology, social work, or other related human services degree.
• One year experience in mental health or human services preferred.
• Valid Colorado Driver’s license and a recent clean driving record.
• Ability to transport the people we serve in your vehicle.
