Clinical Case Manager I/II - High Intensity Treatment Team

Clinical Case Manager provides comprehensive psychological and behavioral case management services to improve or maintain each mental health consumer’s abilities to function in the community. They assess, monitor, and follow up with consumers to meet basic needs and provide transportation to community resources.

Requirements

• Bachelor's or Master's degree in psychology, social work, or other related human services degree
• One year experience in mental health or human services preferred
• Valid Colorado Driver's license and a recent clean driving record
• Ability to transport people in your vehicle
