Ambulatory Care Nurse - RN

Westside Family Healthcare is seeking motivated and compassionate Nurse-Adjusters (RNs) to join their team in a primary care medical home. The role involves managing high-risk patients, coordinating services, and providing patient-centered care, supporting primary care providers, and promoting health and wellness within a primary care medical home. The nurse is a vital part of the team, fostering a supportive environment for patients and ensuring they adhere to their treatment plans.

Requirements

• Graduate of an accredited nursing program with an associate degree in Nursing
• Active RN licensure in the State of Delaware (or compact state)
• Current BLS certification
• Active Ambulatory Care Certification (AMB-BC)
