RN Case Manager - Whittier Rehabilitation Hospital

Whittier Rehabilitation Hospital is seeking a Full-Time RN Case Manager to oversee and coordinate patient hospital stays from admission through discharge, working with the interdisciplinary care team. This role involves ensuring optimal rehabilitation services and coordinating patient and family communication for safe and timely discharge planning.

Requirements

• Registered Nurse with 2-5 years acute care case management experience.
• Excellent communication (written and verbal)
• Basic knowledge of CMS guidelines and Managed Care products
• Ability to assess, analyze, plan, implement and evaluate plan of care.
• Ability to advocate for patients
