Job Description
Provide individual counseling, group counseling and case management services to individuals with Substance Use Disorders (SUD) to achieve sustained remission, restore healthy social connections, obtain stable income and permanent housing, and reduce homelessness risk.
Requirements
- Provide individual counseling, outpatient group counseling, intensive outpatient group counseling, and case management to homeless individuals with substance use disorders.
- Conduct diagnostic assessments, review and sign off on Level I Case Counselor Notes, and provide mentoring to Level I Case Counselors.
- Establish and maintain regular collaborative relationships with social service organizations and programs that residents may utilize for employment, housing, continuing recovery and other needs.
