Job Description
Broward House is seeking a Linkage Specialist to provide HIV/STI/Hepatitis testing, outreach, counseling, and facilitation services to high-risk, vulnerable patient populations. This role is key in connecting individuals to HIV care, prevention, and supportive services within the community and agency. The position requires being driven by improving community and dedicated to client care.
Requirements
- HIV/STD and Hepatitis C screenings
- Engaging, counseling, managing, motivating and mentoring high-risk patients
- Assessment and development of risk reduction strategies
- Data entry and communication with relevant databases
