Medical Social Worker (MSW) - Home Health & Hospice - Northwestern Santa Clara County

We are looking for a Medical Social Worker to provide care at patients' homes in Santa Clara and San Mateo counties. The successful candidate will assist patients and families in coping with problems resulting from severe or long-term illness, and assess, diagnose, and treat patients' mental and social conditions.

Requirements

• Assesses psychosocial status of patient and social/emotional factors related to the patient's illness, need for care, and response to treatment
• Assisting patients and families with goal setting, counseling, and planning and providing interventions
• Assisting patients and families with appropriate community resources, eligibility criteria, and regulations
• Maintains verbal and written communication with other personnel involved with the care of the patient
• Maintains appropriate and timely patient care records in the EMR
