Job Description
American Hospice and Home Health Services is seeking a full-time or part-time Speech-Language Pathologist to provide therapy to patients in Alameda, Contra Costa, Solano, San Joaquin, Stanislaus, and other counties. The role involves evaluation, treatment planning, and implementation of speech, language, swallowing, and cognitive therapy, and should be coordinated with interdisciplinary team members to deliver high-quality patient-centered care. This position offers per-visit pay and an hourly rate, depending on experience.
Requirements
- Perform comprehensive speech, language, voice, swallowing, and cognition assessments.
- Develop individualized, measurable therapy plans.
- Coordinate with nursing, physicians, and caregivers.
- Complete documentation and compliance tasks.
- Participate in care team meetings and interdisciplinary conferences.
- Maintain current licensure and pass CA board exam.
- Valid Speech-Language Pathologist license in California.
