Job Description
The Case Manager Mental Health Professional or Qualified Mental Health Professional (MHP/QMHP) is responsible for developing and implementing individual treatment plans utilizing the IM+CANS to maximize the client's independent community living skills.
Requirements
- Bachelor's degree in Human Services or related field required for MHP; Master's Degree required for QMHP
- Experience working in social services required
- Ability to navigate stairs, no lifting restrictions, and provide services in clients' homes
