Job Description
Cadence Health is seeking a remote-based Nocturnal Nurse Practitioner to serve as a point person for patients navigating their remote care journey. This role focuses on triaging patients based on vitals, managing treatment plans, and collaborating with physicians to optimize patient outcomes, particularly in heart failure, T2D, and hypertension. The company is focused on providing proactive care to seniors by combining technology and clinical expertise.
Requirements
- Active Compact Multi-State RN license
- Seven active Nurse Practitioner (NP) licenses including TX, and six or more of the following states: AL, AR, CT, FL, IL, IN, KY, MI, MS, NC, NY, OK, PA, TN, VA, VT, WI
- Master’s Degree in Nursing or related field
