Job Description
Cadence Health is building a better system to support proactive care for seniors living with chronic conditions. The company seeks a remote-based Nurse Practitioner to triage patients, manage treatment plans, and ensure excellent patient interactions. Requirements
- 5+ years experience treating patients with chronic diseases (T2D, Hypertension, CHF).
- Active multi-state compact RN license.
- ANCC or AANP certification required.
- Master’s Degree (or willingness to expand)
