Job Description
Cadence Health is seeking a remote-based Nurse Practitioner to help patients navigate their remote care journey. The successful candidate will oversee patient vitals, manage treatment plans and medications, and respond to patient escalations.
Requirements
- Active Compact RN license required
- Active NP licenses in CA and WA, and in at least one of: AK, AZ, HI, MT, NM, or OR
- 5+ years experience treating patients with chronic diseases (T2D, Hypertension, CHF)
- ANCC or AANP certification required
- Master’s Degree as a Nurse Practitioner
