Job Description
Care Alliance is seeking a Clinical Outreach Provider (NP/PA) to deliver high-quality primary care to diverse populations, including K-12 schools, homeless shelters, and outpatient treatment centers. The role involves providing both brick-and-mortar and mobile medical services, collaborating with interdisciplinary teams, and focusing on equitable access to care for vulnerable populations. This position emphasizes compassion, collaboration, and accountability.
Requirements
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA) in the State of Ohio.
- Board-certified in Family Medicine, Internal Medicine, or related specialty.
- Current DEA license.
- Two or more years of clinical practice in primary care or community health.
- Commitment to serving diverse, underserved populations.
- Strong collaboration, communication, and problem-solving skills.
- Ability to work in both traditional clinical environments and mobile/outreach settings.
